This extensive questionnaire covers your child's complete health history, including mother's pregnancy, child's birth, allergy/ear infection history, developmental issues and present condition. You will be asked for your goals and desires for your child. This information is crucial in understanding how you may want to proceed with the Voyage into Recovery therapy services.
